To His Excellency.
The President of the United States

This is to certify that I am a Citizen of Lafayette county Twenty five years old. I served six months as a private in the army of the Confederate States & was discharged on account of my health and I avoided conscription for over four years on account of my feeble & weak condition. I have never held any office civil or military under the Government of the so called Confederate States. I have seen your amnesty proclamation and I see none that affects me unless I may be possible be considered worth more than ($20,000) Twenty Thousand ^dollars^ and although I do not value myself so high yet I fear there may be some doubt raised on the subject hereafter and in order to relieve myself from all apprehension or doubts I respectfully ask [?] [favor?] and your permission to take the amnesty oath

I C M Thompson do Solemnly Swear in the presence of Almighty God, that I will hereafter faithfully support protect & defend the Constitution of the United States, and the Union of the States I hereunder [and?] that I will in like manner abide by and faithfully Support all laws and proclamations which have been made during the [existing?] rebellion made in reference to the emancipation of slaves [?] [?] [?]
